Check your registration and confirm that you are eligible to sign for Florida’s 6th Congressional District. A county name alone does not establish eligibility; several counties are only partly within the district.
Choose your county & check with its elections officeUse U.S. Letter paper (8½ × 11 inches), single-sided, at actual size / 100%. The PDF already has two forms. Don’t choose another “two pages per sheet” setting. Check that every border and field is visible.
Follow the marked center cut line. Each half is a complete petition. Use only one half yourself; do not sign both. Keep the candidate name, fields, and form notes attached.
Print your name as it appears on your voter information card. Add your residential address, city, county, state, and ZIP. Provide your date of birth or voter-registration number—not a Social Security number.
Already filled in: Adam James Chancery · Republican · Representative in Congress, District 6. Leave those entries unchanged.
Use an original ink signature and personally enter the date signed in the form’s MM/DD/YY format. Your signature is checked against your voter record. Do not sign for someone else or knowingly sign twice for Adam.
Choose the county where you are registered below. Call to confirm its submission arrangements and office hours, then bring the signed paper original to that county’s Supervisor of Elections. A scan or electronic signature does not replace it.

No. This is a petition to help place a candidate’s name on the ballot. It is not a ballot and does not cast your vote in a primary or general election.
No. Both halves are for the same candidate. Each is intended for a different eligible voter. Do not knowingly sign more than one petition for Adam. If you think you made an error or already signed, ask the elections office before completing another.
No online signing or upload is offered here. Print the PDF, complete the form, sign and date it yourself, and return the original paper. An emailed scan or electronic signature is not a substitute.
The return guide covers all six counties currently included in District 6: Flagler, Lake, Marion, Putnam, St. Johns, and Volusia. Choose your county of registration and confirm eligibility with its elections office. Parts of several listed counties are outside the district, and 2028 requirements need confirmation.
